Rules and Regulations for the Harlequin Dance Scholarship Program
1. Dance students must complete all required information on an official Harlequin Dance Scholarship program application. Additional forms may be obtained by calling 800-642-6440, or by visiting our website at www.harlequinfloors.com. Photocopies of the form are acceptable; and applicants may apply online. Forms may also be faxed to 856-231-4403.
2. Please ensure that the application form is complete so that you are included in the random drawing.
3. Mailed application forms must be returned to: American Harlequin Corporation, Dance Scholarship Program, 1531 Glen Avenue, Moorestown, NJ 08057, and postmarked not later than November 1, 2012
4. Employees and family members of employees of American Harlequin Corporation, British Harlequin plc, and Harlequin Europe SA are ineligible to apply and/or receive scholarship monies.
5. U.S. and Canadian citizens between the ages of 15 and 21 years of age are eligible to apply for these scholarships. Applicant must be at least 15 years of age by November 1, 2012, or not older that 21 years of age by November 1, 2012. Only one application per name, please!
6. Scholarship winners will be randomly selected before witnesses at the annual American Harlequin Corporation Corporate meeting to be held in December 2012. Winners will be notified by mail and the names of the scholarship winners will be announced on our website after their eligibility has been verified.
7. One (1) $5,000.00 Scholarship, two (2) $3,000.00 scholarships, three (3) $2,000.00 scholarships, two (2) $1,000.00 scholarships, and twelve (12) $500.00 scholarships will be awarded not later than December 31, 2012.
8. Applicants must be enrolled in a public or private dance school program
at the time of the application.
9. American Harlequin Corporation reserves the right to withhold scholarship monies pending verification of the dance student's eligibility. No applicant is considered to be a scholarship winner until the verification process is complete. This process requires that after selection, applicants under the age of eighteen (18) years must have the signature of a parent or legal guardian to be verified.
10. Once selected as a winner, any applicant who does not return the required verification information by the deadline date established by American Harlequin Corporation, will be disqualified, and a new winner selected to receive the scholarship in his/her place.
